We integrate and coordinate all primary, acute, behavioral health, long-term services, and supports to treat the whole person. Integrated care is available for children and adults with Medicaid/Soonercare who have a serious mental illness/serious emotional disturbance AND a chronic health condition or are at risk for chronic health conditions.
